At AZ Mobile Pet Vaccines, we provide every client with their pet’s medical records at the time of service and offer replacement options when needed.
📍 At Your Visit
You will receive a yellow carbon copy medical record at the time of your visit.
This is your pet’s official medical record, and we strongly recommend keeping it in a safe place or taking a photo for backup.
📄 Need a Replacement Record?
If your original record is lost or you need another copy, we’re happy to help.
All replacement records are sent via PDF email.
💲 Record Request Fees
- $10 – Single visit medical record replacement
- $25 – Complete medical history (all available records)
These fees help cover the time required to locate, verify, and process records from our system.
⏱️ Processing Time
- Standard requests: 24–72 hours
- Older or archived records: up to 5 business days

Please include:
- Owner’s full name
- Pet’s name
- Approximate date/location of visit (if known)
⚠️ Important Information
- Your yellow carbon copy is your primary record
- Requests must be made by the pet’s legal owner or with written authorization
- Records are released in accordance with the Texas Board of Veterinary Medical Examiners
